Title:Telecare and telehealth converge in the Adriatic Region

Description:The proposed project will involve collaboration between local governmental bodies and Primary/Community Health Care Services, working together to deliver integrated health and social care remotely to patients who live in remote areas and those who are unable to regularly visit the health care unit (e.g. elderly, disabled, without family members in vicinity, etc). The technology and scientific expertise on remotely delivering health care services will be achieved by using Research and Medical Institutes as the scientific and technology arm. The proposed initiative aims to overcome the divide between health and social care provision in the Adriatic region. In this context, the main objective of the project is to take advantage of the use of tele-health technology for chronic heart failure (CHF) and Chronic Obstructive Pulmonary disease (COPD) patients to reduce unnecessary admissions to hospital as well as facilitate early supported discharge after a hospital visit.

Partners searched:Partners from the other participating countries (Slovenia, Croatia, Bosnia and Herzegovina, Montenegro, Serbia) - Ideally Local Governmental Bodies & Primary/Community Health Institutions
